About The Position

Since our opening in 1987, New Hope Treatment Centers has been a welcoming place for young people in moments of crisis. Our programs have played a role in countless success stories, thanks to our relational approach to behavioral healthcare. The Case Manager is responsible for coordinating, monitoring, and documenting individualized services for assigned residents to support treatment goals, continuity of care, and regulatory compliance for the Dillon Department of Juvenile Justice in Butner, NC.

Requirements

  • Bachelor’s or master’s degree in social work, Counseling, Psychology, Human Services, or a closely related field from an accredited institution.
  • Must meet the requirements of a Qualified Professional (QP) in the state of North Carolina.
  • Valid driver’s license and acceptable driving record, if transportation or community coordination is required.
  • Ability to pass required background checks, drug screening, and any state-mandated employment clearances.
  • Strong interpersonal and communication skills.
  • Ability to manage multiple priorities and maintain organized records.
  • Ability to work independently and as part of a multidisciplinary team.
  • Professional judgment, discretion, and ethical conduct.
  • Proficiency with electronic health records and standard office technology.
  • Work is performed in a behavioral health treatment environment that may involve exposure to emotionally distressed individuals.
  • Ability to sit, stand, walk, bend, and use standard office equipment.
  • Ability to respond to crisis situations, which may require quick movement.

Nice To Haves

  • Experience working with children, adolescents, or individuals in behavioral health, residential treatment, or human services settings.
  • Knowledge of service planning, care coordination, and clinical documentation standards.
  • Familiarity with North Carolina behavioral health regulations and service systems.

Responsibilities

  • Coordinate and manage individualized service plans for assigned residents in accordance with clinical, organizational, and regulatory requirements.
  • Monitor resident progress, conduct routine reviews, and communicate updates to the interdisciplinary treatment team.
  • Collaborate with therapists, medical providers, families, external agencies, and community resources to ensure continuity of care.
  • Advocate for residents by facilitating access to appropriate clinical, educational, medical, and social services.
  • Maintain accurate, complete, and timely documentation in compliance with applicable state regulations, licensing standards, Medicaid requirements (if applicable), and organizational policies.
  • Facilitate treatment team meetings, discharge planning, and service coordination activities.
  • Maintain professional boundaries and adhere to confidentiality requirements, including HIPAA regulations.
  • Perform other duties as assigned consistent with the nature of the position.
