About The Position

The Permanent Supportive Housing Case Manager provides trauma-informed, family-driven case management services to unhoused families as they transition from homelessness into stable housing throughout Salt Lake County. This role works closely with each family to assess their unique goals, identify needs, and address barriers to self-sufficiency. The Case Manager connects families to vital community resources and partners with them to develop personalized plans that support long-term housing stability and overall well-being. This position requires a deep commitment to serving families with compassion, respect, and a strengths-based approach that empowers them to thrive in their new homes.

  • Ability to lift and move heavy items up to 25 pounds.
  • Ability to sit, stand, and move for at least an hour at a time or more.
  • Must be at least 21 years old, have a valid, unexpired driver’s license, and the ability to drive a vehicle, as needed.
  • Must pass a pre-employment background check and drug screening.

Nice To Haves

  • Experience working with diverse and/or vulnerable populations is preferred.
  • Lived, work, or volunteer experience in a related field OR a degree in a social services or human services field is required.

Responsibilities

  • Provide trauma-informed case management services to families, including outreach, ongoing assessments, goal setting, and creative problem-solving using a network of community resources.
  • Meet with families in their homes, at emergency shelters, and in other community locations to assess needs and collaboratively establish goals.
  • Assist families in identifying barriers to stability and connecting them with appropriate community resources and support services.
  • Demonstrate a strong belief in the Housing Focused model and engage families in ongoing, supportive conversations about housing.
  • Help families obtain income through employment, benefits, and other resources to support long-term stability.
  • Assess children's needs and make appropriate community referrals, including liaising with schools to coordinate supportive services.
  • Focus on housing and strength-based practices to meet the unique needs of each family.
  • Collaborate daily with all departments to promote a supportive and effective team environment.
  • Build and maintain relationships with community partners to identify housing options and additional resources for families.
  • Attend weekly team meetings and actively contribute to case-conferencing discussions.
  • Maintain accurate records, detailed case management notes, and required statistical data; participate in special projects as needed.
  • Collaborate with community partners and agencies to support families experiencing mental and physical health needs.
  • Use reliable transportation to travel to appointments and meetings.
  • Attend mandatory agency trainings, including Trauma-Informed Care, Housing Focused, De-escalation, Suicide Prevention, CPR, and more.
  • Participate in emergency drills and environmental safety activities, as required.
  • Follow all agency protocols for security, safety, and sanitation to maintain a clean and secure environment for guests and staff.
  • Maintain regular and reliable attendance as an essential function of this position.
  • Other duties as assigned.
