About The Position

Arms Acres is looking for a Clinical Case Manager to join our team in Carmel, NY. In this role, you will provide comprehensive clinical case management services that include: assessments, individual, group and family treatment, treatment plan coordination, referent relations, discharge planning, and intervention with client support systems, including social service programs and managed care. sign on bonus eligible

Requirements

• CASAC-T, CASAC, MSW with limited permit, LMSW, LCSW or LMHC
• Experience working with substance abuse counseling preferred

Responsibilities

• Provides/ensures comprehensive evaluation of patient's functioning as defined by facility policy and procedures.
• Coordinates the development and implementation of the multidisciplinary treatment plan; coordinates treatment plan review; performs these activities within established time frames.
• Documents patient progress and own interventions in case record and works within time frames of regulatory requirements.
• Provides individual, group and family treatment (psycho-education, life skills) as identified in patient treatment plans and requested by program director.
• Performs utilization review, provides progress reports and develop aftercare plans in conjunction with referring persons/agencies, EAPs and managed care/insurance entities.
• Participates in facility program and quality improvement activities.
