About The Position

The Case Manager will conduct intake interviews, care plan development, care coordination, referral services, substance use counseling coordination, health education, and social services navigation to members with substance use and other chronic health conditions. Other interventions are Healthy Conversations, AOD Counseling and Seeking Safety education as eligibility to deliver them allows. The Case Manager will act as an intermediary between providers and members – serving in numerous roles such as educator, evaluator, service coordinator, community resource researcher, and more. This role will work with Ryan White and Harm Reduction prevention services for high-risk of HIV populations. This position is scheduled for 9AM to 5PM from Monday to Friday

Requirements

  • Preferably a bachelor's degree in human services field with at least two years of case management or social services experience.
  • Minimum of a High School diploma with no less than 5 years of proficiency in case management and/or care coordination services experience.
  • Ability to make data entries into programs database such as AIRS, e-Share, EiCare
  • Good group facilitation skills and overdose prevention training experience
  • Skilled with health insurance navigation and documentation.
  • Good verbal, written, communication and interpersonal skills.
  • Use of computer software --Microsoft Office Suite including Word and Outlook calendaring, and Excel.
  • Culturally aware and sensitive to PWUD (People Who Use Drugs), PLWHA (People Living with HIV/AIDS), LGBTQAI, YIDU (Young Intravenous Drug Users), SMI (Severe Mental Illness), and MSM (Men who have Sex with Men)
  • Minimum of 2 years of experience of case management with marginalized and high-risk populations.
  • Experience working with the adult population who has co-occurring substance use and non-chronic mental health disorders.
  • Knowledge of community health and social service agencies, public benefits and additional community resources
  • A good understanding of what Harm Reduction and how it is used in case management.
  • Frequently remaining in a stationary position, often standing, or sitting for prolonged periods
  • Occasionally moving around to other worksites
  • Occasionally lifting and moving objects up to 25 pounds in ordinary office related tasks.

Nice To Haves

  • Bilingual in Spanish/English preferred

Responsibilities

  • Completes initial assessment as needed and reassessments on all assigned clients in a timely manner in-order to develop individualized treatment goals for each client.
  • Establishes treatment goals and objectives with clients as part of the interdisciplinary team.
  • Provides case management services including but not limited to weekly and monthly progress summaries, quarterly assessments, quarterly care planning, counseling, outreach, case conferences, advocacy, referrals (on-site & off-site) and discharge planning.
  • Facilitates harm reduction individual and group services as needed.
  • Attends weekly/bi-weekly supervision meetings as agreed upon by the Program Coordinator.
  • Ensure timely submission of clinical documentations to care organizations for uninterrupted services.
  • Coordinate transitional care from inpatient service to other settings as per care plan.
  • Coordinate detox referrals and transportation services as needed.
  • Conduct prescreening and intake assessment interventions with eligible individuals as needed.
  • Able to educate participants on Harm Reduction intervention and services.
  • Conduct overdose prevention education and Narcan dispensation as needed.
  • Work collaboratively with team members to provide outreach and engagement too hard to reach clients.
  • Maintain documentation and update care plans at specific time intervals or as needed in Housing Works and Health Home electronic medical records system.
  • Assists clients to navigate the health insurance system including the AIDS Drug Assistance Program (ADAP) and/or other insurance networks.
  • Coordinate with supervisor escorts to life saving services and home visit as needed.
  • Conduct field work and other provider settings as needed to fulfill goals in the plan of care.
  • Able to meet assigned deadlines for progress note documentation and service coding according to program Scope of Services.
  • Participate in regular case conferences with medical providers and other stakeholders as appropriate to the care and treatment of the client.
  • Aid through advocacy and psychoeducation to clients and family for entitlements and other community services.
  • Understands and is sensitive to chronic medical conditions, including HIV, mental health and substance use, diabetes, asthma, hypertension, heart disease and hepatitis diseases.

  • Participate in quality assurance and quality improvement planning project activities.
  • Participates in monthly staff development meetings and interventions.
