Requirements

  • Bachelor's Degree in Social Work or related field from an accredited college or university required.
  • One year of case management experience in a social service or healthcare setting required .
  • Demonstrated ability to render adequate service to clients and to organize and manage job efficiently.
  • Must have access to a vehicle, valid driver’s license and proof of insurance to perform essential job function, required.
  • Ability to maintain assigned community and home-based services.
  • Ability to work effectively with diverse individuals and groups.
  • Satisfactorily pass all relevant screening and criminal background checks.
  • Driving for work required with transportation of clients in a personally owned vehicle.
  • Travel between sites required.
  • While performing the essential duties of this job, the employee is required to sit and stand for sustained periods of time, frequently talk, hear, use hands and fingers to feel, handle and operate objects or controls, and occasionally stand, bend, stretch, stoop, kneel and crouch.
  • The employee is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing and viewing a computer terminal.
  • The employee must be able to walk up and down stairs to clients' apartments.
  • The employee may occasionally lift and/or move up to 25 pounds.

Responsibilities

  • Conducts comprehensive client assessments for participants.
  • Develops a written service plan with the client to meet client needs, specifying goals to be achieved and services to be provided and supplements/service plans to meet client needs.
  • Provide individual mentoring services.
  • Supplements/service plans to meet client needs.
  • As appropriate, provides advocacy, case management, and supportive counseling or referrals to counseling for individuals, families, and groups in coping with environment and interpersonal relationship issues.
  • Uses expanded knowledge of resources and helps clients utilize them.
  • Works collaboratively with community resources.
  • Handles emergency and crisis situations within target population.
  • Works within contract, grant, and program requirements including meeting timelines and reporting requirements.
  • Monitors client situation and adequacy of service plan.
  • Assess and monitor client situation and adequacy of service plan.
  • Collaborates and participates with other program staff in the planning and implementation of service plans.
  • With supervisory approval, authorizes limited amounts of financial assistance to own clients.
  • Identifies systems issues and reports to Program Supervisor as appropriate.
  • May work with community groups and may carry a special community assignment.
  • Takes increasing responsibility for own professional development, utilizing supervision and other developmental opportunities within and outside the agency to enhance skills and understanding.
  • Responsible for orientation, intakes, detailed individual service plans, timely case management, wrap around services and data entry into the database system.
  • Case management service could be provided in office, client home, and community-based settings.
  • Up to 10% other duties as assigned by supervisor.
  • Works effectively with diverse staff and service populations.
