About The Position

The Case Manager is responsible for performing client care activities as directed by the Clinical Director. The Case Manager is responsible for managing an assigned caseload of clients having substance use disorders, for assessing client needs, and for coordinating basic needs services for clients from admission through discharge. The primary focus is job seeking assistance and housing assistance for clients. The Case Manager collaborates with the Treatment Team in the formulation of the Discharge Plan,and assists in coordinating discharge planning.

Requirements

EDUCATION: Graduate from a college or university with a Bachelor’s Degree in human services or health-related field. Will consider non-degreed individual with at least 1 year of similar case management experience.
EXPERIENCE: A minimum of 1 years’ experience with chemical dependency - psychiatric setting preferred
LICENSURE/CERTIFICATION: Certification in Case Management or substance abuse is preferred. Current CPR certification and First Aid.

KNOWLEDGE AND TRAINING REQUIRED AT TIME OF HIRE: Knowledge of the Principles and Practices of the discipline. Knowledge of Joint Commission Standards. Demonstrates Proficiency in Communication & Written skills. Knowledge of State & Federal Statutes Regarding Client Confidentiality laws. Knowledge of Drug-Free Workplace Policies. Knowledge of Workplace Violence.

Responsibilities

Provides discharge planning with all clients upon admission through discharge and will complete discharge planning/discharge paperwork under case management.
Makes appropriate referrals to access benefits, vocational services, legal services, etc.
Coordinates with necessary employers, legal entities, and other appropriate parties based on client needs.
Track client progress and outcomes oversees alumni and university interns.
Note scribing for Clinical Director and as needed for other therapists.
Provides progress reports or letters as needed within the limit of signed releases of information that are active and on file in the client record.
Assists clients in finding jobs and sober living accommodations upon discharge.
Aware of and uphold HIPAA, Confidentiality, and all other local, State and Federal regulations that applies to their position and protecting client confidentiality and privacy to the fullest extent of the law.
Ensures that all individuals are adequately and appropriately served according to their individual needs
Participates in regular interdisciplinary staff meetings held on-site.
Provides on-going supportive and/or case management functions in accordance with the problems, needs, and the strategies identified within the client treatment and discharge plan in order to help the clients to achieve the stated goals and objectives
Provides face to face reviews with the client and/or relevant staff on a regular basis the progress made in reaching service goals so that the service plan can be modified as necessary to ensure that the goals and objectives are being achieved. The frequency reviews will be determined by relevant requirements.
Documents all service contacts on a timely basis including face-to-face interviews, collateral and networking contacts, correspondence and maintains the case records in accordance with agency and regulatory standards and requirements.
Participates in interagency planning and service coordination activities as directed to improve and enhance service continuity and effectiveness for clients to include but not be inclusive of referrals to interagency programs, including transition/transfer to other programs and/or sober support networks.
Meets regularly with the immediate supervisor as a means of enhancing professional growth, reviewing and processing the provision of case management services, and dealing with appropriate administrative issues.
Maintains close communication with the executive team, clinical team and residential staff as needed.
Maintains client belongings/contraband in a locked cabinet from admission through discharge.
Performs other duties as assigned by the supervisor which are consistent with the position and in compliance with agency policies and procedures.
Documents as required in client chart; notes are expected to be completed on the same day as services are delivered, no more than 24 hours later than the service delivered.
Adheres to policies and procedures.
Performs other duties as assigned.
