About The Position

The Care Management (CM) Case Manager is a member of the interdisciplinary care management team and is responsible for providing support to patients and families, and technical or caseload support to the care management team. This individual will demonstrate strong computer skills and an aptitude towards learning multiple computer applications. The case manager will be instrumental in providing support to the care management team by assisting with documentation within the electronic record and any other database pertaining to provision of care management services. The CM Case Manager will be responsible for conducting outreach and engagement activities to inform potential clients of eligibility for the Health Homes program. As directed by the Care Manager (or designee) the CM Case Manager will meet with potential clients in their home, community, or clinic, and will assist them with understanding services available through the Health Homes and Sea Mar Identified Care Management programs . As an advocate for those with chronic conditions, this person will have a general understanding of common chronic conditions, and the emotional issues that can come from these conditions. The case manager has an ability to work on behalf of the client under the direction of the Care Manager, and support the client in working towards goals set with the Integration Specialist or Care Management RN The CM Case Manager works with healthcare professionals within a variety of organizations (e.g. hospitals, government agencies, insurance plans) in order to help coordinate services for clients. They are aware of the scope of his or her own expertise and that of others, and works well within those parameters.

  • The person in this position must be of good character and able to maintain the understanding, physical health, emotional stability and personality suited to meet the physical, mental, emotional, and social needs of the population served.
  • Must have good organizational and communication skills and demonstrate professionalism and appropriate boundaries in all interactions.
  • The person in this position shall have no history or evidence of alcohol or other drug misuse for a period of three (3) years prior to the date of employment at the facility, and no misuse of alcohol or other drugs while employed at this facility.
  • This individual cannot be a person who has been convicted of a felony within the last seven years or ever been convicted of assault, abuse, fraud, or crimes that have brought harm to another financially, emotionally, or physically.
  • This person must have a valid driver license, proof of auto insurance and a vehicle safe for daily use. Prior to hire this person must submit a driver’s abstract demonstrating that s/he is safe to drive as deemed by Sea Mar’s Health and Safety/Compliance Officer. Within the course of employment, a current driver’s abstract may be requested.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write routine reports and correspondence.
  • Ability to speak effectively before groups of customers or employees of organization.
  • Must be a High School Graduate or equivalent; Bachelor’s degree preferred.
  • Must have experience working with people with diverse cultural and socio-economic backgrounds
  • This position must obtain Basic Life Support (BLS) CPR within 90 days of hire date and is required to maintain current BLS CPR throughout employment.
  • Must have a valid driver’s license and proof of auto insurance.
  • Must complete a pre-hire and annual TB test screening.

  • Annual influenza vaccine required. Only exception is for employees with a medical or religious exemption approved by Administration. Employees with an approved medical or religious exemption must wear a mask at all times during the flu season.

Nice To Haves

  • Prefer education and/ or experience with working with individuals with chronic conditions.
  • Prefer one year paid or volunteer experience providing medical, social service or behavioral health case management or care coordination.

Responsibilities

  • Assists with maintaining records and assists with administrative work as assigned by Care Manager.
  • Provides care coordination support to assigned patients. These services may include linking patients to Sea Mar CHC and community resources, or directly providing assistance to the patient to obtain and/or maintain housing, income, employment, medical and behavioral health treatment, activities of daily living, and meaningful social activities in the community.
  • Conducts outreach and engagement activities via telephone and in person Develops knowledge of community resources (financial, vocational, social, cultural, spiritual, etc.) and methods to help patients to access services and resources as appropriate.
  • Advocates for patients across systems.
  • Participates in case consults with care management team.
  • Attends and participates in other agency or community meetings as directed.
  • Collaborates with clinic staff, to coordinate care for Health Home/ Care Management clients.
  • Keeps the Care Manager and team informed of any potential problems in a timely manner.
  • Completes daily documentation of all patient care management activities performed.
  • Performs any other duties or special projects as assigned by Care Manager.
  • Maintains quality professional standards in all areas of work, so that the goals of the agency can be achieved.
  • Follows Sea Mar safety procedures, including required tests and drills.
  • Follows all of Sea Mar’s building security procedures.
  • Presents a professional image to all patients, the public, and staff at all times, both in person and on the telephone.
  • Understands HIPAA compliance measures and maintains strict confidentiality in all work related areas.
  • Handles all patient information and patient activities in a confidential manner in keeping with Sea Mar policies.
  • Participates in staff and program meetings, including consultation, supervision, and professional training sessions, as required by Care Manager.
  • Maintains all records, reports and forms in accordance with Sea Mar procedures and contract requirements in a timely manner; performs the duties required by Sea Mar policies.
  • Holds a valid Washington State Driver’s License and owns or has access to an insured vehicle.
  • Must be comfortable yet professional in the client’s home setting with ability to set and maintain personal boundaries.
