Program: Valley's Residential Programs Pay: dependent on certification status (see below) Uncertified Case Managers start at $17.50 - Uncertified Case Managers will obtain certification within the first 90 days and pay will be increased Certified Case Manager range starts at $19.61 - Certified CM pay is calculated based on years of certified experience. Shift Differential: additional $2.00/hour when clocked in for 3+ hours between 3pm-11pm Schedule: PRN Shifts | Day shifts/Swing shifts as needed This role will be an as needed Case Manager for Valley's EPIC and Phoenix residential programs. Job Summary & Deliverables Case Managers provide psychosocial rehabilitative services as indicated by the client’s care plan to support clients in meeting their individualized goals. Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities, and ensuring that regulatory compliance and organizational standards are met.
