Requirements

Bachelor's or Master's degree in a related field preferred (e.g., Social Work, Nursing, Psychology)
Clinical case management experience, preferably in a rehabilitation or healthcare setting
Strong interpersonal and communication skills
Knowledge of brain injuries and rehabilitation preferred

Responsibilities

Work with insurance and payers to advocate for the appropriate rehabilitation and medical care.
Conduct comprehensive assessments of patients' medical, social, and psychological needs
Collaborate with the treatment team to develop individualized care plans.
Coordinate and monitor the delivery of services, including therapy, medical care, and support services
Advocate for patients' rights and ensure their needs are met throughout their rehabilitation journey
Provide education and support to patients and their families
