About The Position

As the Case Manager you will support the Manager for the Transitional Housing Program and program residents to ensure the coordination, 24-hour operation, and program implementation of the Transitional Housing Program (THP).

Requirements

  • Demonstrated skills in program management, supervision, and financial management.
  • Demonstrated ability to exercise good judgment.
  • Experience working with diverse populations, such as women and their families and communities of color.
  • Ability to work with community groups and volunteers.
  • High school diploma required.
  • Bachelor’s Degree in related field such as Human Services, Counseling, or Social Services or at least 5 years of related experience.
  • Must pass criminal background check.
  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 15 pounds at times.

Nice To Haves

  • Experience in the field of Substance Abuse Treatment and Recovery preferred.
  • Bi-lingual (English/Spanish) preferred.

Responsibilities

  • Responsible for promoting and providing a residential program environment that coincides with the YWCA Mission.
  • Develops goals and outcomes with individual residents.
  • Provides case management support for individual residents as needed, assists with problem solving, and provides support for health, legal, and advocacy issues.
  • Assists with Department of Transitional Assistance (DTA) benefits, housing and Mass Health applications.
  • Interfaces with all YWCA departments, advocating for residents’ needs.
  • Works with housing providers and community agencies to enhance program services and program participants’ access to community resources.
  • Maintains residence database and prepares program reports.
  • Performs other duties as requested by Manager.
