About The Position

Assist in the coordination of care and services for at-risk CalOptima Health members with multiple medical and psychosocial needs in the Enhanced Care Management program. Work collaboratively with managed care plans, medical providers, social workers, community partners, and other members of interdisciplinary teams to ensure the delivery of safe and efficient care.

Requirements

  • Must be bilingual in Spanish or Vietnamese and at least 21 years of age
  • 4 years relevant experience OR AA degree and 2 years relevant experience OR Bachelor’s degree and 1 year case management experience
  • Experience working with families and youth
  • Reliable transportation, safe driving record, CA driver license, 2 years of driving experience, and automobile insurance required
  • Position may require standing/sitting for extended periods of time, dexterity of hands and fingers to operate office equipment, reaching overhead, above the shoulders and horizontally, bending at the waist; seeing to review and analyze reports, documents or other reports; hearing and speaking to communicate and provide information to others; speaking to be heard before groups of people; position may require to lift, carry, pull or otherwise move objects between 10-20 pounds.

Responsibilities

  • Conduct comprehensive intakes and assessments with clients and families
  • Develop comprehensive care plans by evaluating client needs, setting measurable goals, and conducting regular reassessments to ensure progress and relevancy
  • Manage client care in collaboration with other health professionals and members of client care team in order to make decisions about treatment and meet health goals
  • Provide community-based referrals and resources to clients and families that meet client needs
  • Coordinate discharge planning by working closely with hospital staff to ensure smooth transitions of care and continuity of support post-discharge
  • Assist clients in determining eligibility for and access to County, State, and Federal programs, and obtaining necessary documentation
  • Assist in creating and terminating files, completing all necessary documentation, and updating and documenting all client information and services rendered into database
  • Complete daily process notes for each client worked with and enter information into database
  • Be resourceful and possess a knowledge of social services and support community based mental health and family services
  • Participate in group, clinical, and multi-disciplinary case consultations when necessary
  • Attend staff/volunteer meetings and trainings
  • Ensure a productive work environment by working as a team member, contributing to team meetings and providing feedback to support decision making
  • Contribute to the quality of a high performing team and take on different roles as needed; continuously manage own time and resources
  • Be compassionate and committed to helping children and families reach their full potential; work with diverse populations in a culturally competent manner and work one-on-one with youth

  • Maintain close, daily contact with staff, clients and supervisors to receive and provide information, discuss issues, explain or interpret guidelines and instructions
  • Effectively and with proficiency, use Microsoft Office applications - specifically Outlook, Excel, and Word, electronic scheduling, and database software
  • Ensure the ARCHES office meets health and safety standards and ensure the safety of all clients
  • Take action to prevent accidents and respond to injuries and signs of illness
  • Utilize strong organizational skills with attention to detail, follow-up, and manage demanding caseload
  • Prioritize and complete multiple tasks within expected deadlines; adapt and be flexible
  • Utilize strong communication skills - both written and verbal, and provide feedback regularly; speak in front of large and small groups
  • Utilize excellent documentation skills with demonstrated ability to write clearly, concisely, and descriptively
  • Maintain knowledge of HIPAA and confidentiality regulations; follow organizational procedures
  • Maintain experience in case management and knowledge in case management philosophy and function
  • Maintain skills and ability in project management, providing training, networking, and establish collaborative relationships
  • Maintain knowledge of administrative and clerical procedures and systems such as managing files and records, following up on phone calls, receiving and documenting payments, conducting meetings, completing reports, and tracking data
  • Attend trainings regarding Medi-Cal and CalAIM programs, including Enhanced Care Management and Community Supports updates regarding changing requirements
  • Act as a patient advocate for clients requiring assistance with access to healthcare, behavioral health, and other needs
  • Complete all documentation per Medi-Cal, Orange County, and BGCGG standards within expected deadlines
  • Arrange transportation for clients to access health services
  • Encourage health prevention and awareness
  • Participate in group, clinical, and multi-disciplinary case consultations
  • Provide information to and screen potentially Medi-Cal eligible children and family members to obtaining Medi-Cal health care coverage
  • Assist families in applying for Medi-Cal, if applicable, and discuss follow-up health care
  • Actively encourage parents regarding enrolling uninsured children in health insurance, including Medi-Cal/Healthy Families
  • Support with ARCHES food pantry, including but not limited to receiving deliveries, placing and coordinating food orders and purchases
  • Assist in screening, selecting and enrolling clients, and follow up with phone contact as needed
  • Attend coordination meetings, case management, and informational meetings in order to maintain and develop community partnerships; attend other meetings as necessary
  • Effectively support the planning, development, and implementation of ARCHES programs, services, and activities
  • Effectively monitor and evaluate programs, services, and activities through tracking referrals, phone calls, customer service, and any additional responsibilities; assist supervisor with training and supervision of volunteers
  • Continuously develop own knowledge and skills in each of the various Community Health Services programs
  • Assist in outreach activities at health fairs, community events, and other special events
  • Maintain contact as needed with external community groups, schools, members, BGCGG partners, and others to promote and ensure information and referral services for the Community Health Services Programs are successful
  • Report all child abuse cases to CAR and completes reports as necessary
  • Act as a translator when interpretation is needed for services and activities related to case management and facilitating Medi-Cal applications
