Our Shelter Case Managers will provide strength-based case management services to families experiencing homelessness; conduct intensive biopsychosocial assessment of family unit and individual needs; collaboratively develop and implement a comprehensive, individualized housing plan with aftercare services, if appropriate; assist with connecting family to community resources including health, mental health, substance treatment, preventive services and employment services; move families out of shelter/hotel in less than 30 days.
