About The Position

The Case Manager is responsible for providing direct client care under the supervision of the Clinical Director. This role involves maintaining a safe and efficient working and treatment environment per company policies, procedures, and best practices, and communicating effectively with the treatment team to ensure safe, quality care is provided to all clients. Key functions include providing direct support services, engaging clients in case management and personalized aftercare plans, and managing intake documentation. The Case Manager prepares initial aftercare plans, event notifications, and other client-related documentation in accordance with requirements and company policies. Daily monitoring of caseloads is essential to ensure timely completion of documentation such as assessments, case reviews, discharge plans, legal documents, FMLA, and Short-term disability. The position also facilitates coordination of care for clients' initial aftercare plans and requires resourcefulness in seeking appropriate resources for clients and maintaining relationships with outside entities.

Requirements

  • Knowledge of software programs for personal computer use, especially word processing, email, and spreadsheets.
  • Knowledge and experience in case management with substance abuse clients.
  • Ability to prepare, implement, and analyze aftercare plans.
  • Strong problem-solving skills and the ability to carry out assigned projects to their completion.
  • Strong interpersonal skills, including the ability to communicate effectively both verbally and in writing, establishing, and maintaining effective working relationships, gaining cooperation, and resolving conflicts.
  • Ability to plan and organize case workload, manage deadlines of him/herself.
  • Ability to maintain and demonstrate ethical professional standards and demeanor, including the confidentiality of employee and client data, client boundaries, and other sensitive information.
  • Demonstrated competence with, and continued commitment to respect for cultural differences and diversity, protecting client rights.
  • Recognition of the limits of one’s own professional expertise and the willingness to seek supervision and/or support, as needed.
  • Makes clinical and organizational decisions with the client's well-being and best interests in mind.
  • Recognize, prevent, and respond to a situation in which the client may be a danger to himself or others, displaying aggressive behavior or is experiencing a crisis or medical emergency.
  • A high school diploma or a high school Equivalency Diploma (GED).

Responsibilities

  • Provides direct support services to include engaging clients in case management and personalized aftercare plans.

  • Ability to be resourceful; seek appropriate resources for clients and maintain relationships with outside entities.
