About The Position

The Care Manager acts as the key contact for managing and coordinating medical, educational, and behavioral health services for young people and their families. They work on resolving access issues, building strong connections with service providers, and tracking progress, as well as any barriers to care. By working closely with each family and care team, the Care Manager makes sure that all providers are aligned in their dedication to the youth's well-being, helping to identify and leverage their strengths, thus empowering them to succeed both at home and in the community.

Requirements

• Health Home Care Managers who serve High acuity enrollees must have the following minimum education and experience:
• A Bachelor's of Arts or Science with 2 years of relevant experience working with youth and families in child welfare, developmental disabilities, mental health, healthcare, and/or other systems, as well as those receiving preventive services.
• A Master's Degree with one year of relevant experience.
• Valid driver's license and a registered operating car to be used for daily transportation for work-related purposes
• Bilingualin Spanish with bonus
