About The Position

The Case Manager in the Working Wellness Program works collaboratively with our Employment Specialist, Peer Support Specialist, and Clinician to provide case management, mental health services, and employment support services based on the IPS model. The Case Manager is responsible for supporting intakes, assessments, employment services, and ensuring effective client engagement from referral to discharge. This is an action-oriented, community-based program that aims to support clients to find meaningful employment that speaks to their interests and skills, while also addressing mental health needs so that clients can thrive. This role will uphold and champion the agency’s mission, vision, and values and contribute to a collaborative and inclusive work culture. This position is covered under the SEIU 1021 Collective Bargaining Agreement. In addition to the established base pay, members will receive negotiated annual increases as follows: January 2027: 3% base pay increase Future increases will be subject to the terms of the collective bargaining agreement.

Requirements

  • BA degree in the field of psychology, counseling, or social work and two years of experience in the mental health field or four years of experience in the mental health field, or similar education/experience.
  • Experience working with Severe Mental Illness (SMI) populations and principles of case management, individual rehabilitation, family supports, supported services (housing, employment, education), and crisis interventions.
  • Demonstrate a capacity and commitment to work in cooperation with a diverse team of professionals in regards to race; class; gender; sexuality; disability; age; immigration/citizenship status; religion/spirituality; and other social statuses.
  • Ability to organize and prioritize duties as well as multi-task in short periods of time in regards to shifting the aforementioned roles throughout the day (i.e. community outreach, client appointments, writing case notes, IHOT meetings, consultation.)
  • Demonstrate a competency of writing skills effective enough to communicate client services rendered as well as coordinate casework with IHOT staff.
  • Demonstrate a capacity to collaborate in the context of team work, share casework, and communicate effectively so that program operations are executed with minimal challenges.
  • Demonstrate a commitment to be teachable as well as receive and integrate feedback into work performance within an effective manner of time.
  • Demonstrates and models excellent “customer service” orientation and ability to work with diverse populations.
  • Exercises extreme discretion with the ability to hold, balance, and transmit public and confidential information needed by various parties.
  • Proficient in MS Office and Google Workspace products.
  • Valid CA Driver’s license required.

  • Ability to safely operate a motor vehicle
  • Ability to push, pull, and lift up to 50 pounds frequently.
  • Ability to stand and move from one location to another daily.

Responsibilities

  • Perform regular assessments of clients' needs in areas such as: housing, counseling, coordination of services, social relationships, family relationships, job skills, symptom management, navigating entitlement programs, budgeting, and other related areas.
  • Provide case management duties for clients and connect them to various resources as necessary, including, but not limited to, assisting clients in finding housing, mental health services, resources to address food insecurity, childcare, and other supports.
  • Support the team in meaningfully engaging with all clients a minimum of 2 times a month.
  • Provide employment services to clients based on the IPS model, including conducting employer contacts with or on behalf of clients.
  • Provide services in the field at least 50% of the time, including at various institutional settings and locations in the community, as necessary.
  • Develop individualized care plans for clients in collaboration with the team.
  • Provide services related to supporting mental health and skill building to support client’s increased functioning.
  • Collaborate with the Program Supervisor in facilitating weekly team meetings where client cases are reviewed and where the team provides updates and feedback to one another.
  • In collaboration with the Program Supervisor, support team in planning and collaborating in coordination of care and services to a caseload of 50+ clients, including updating and reviewing electronic records to inform client engagement.
  • Collaborate with Lead Clinician and team in planning and facilitating workshops and outreach activities to the community
  • Provide feedback to Program Supervisor about program needs and morale; assist Working Wellness staff to troubleshoot and problem-solve client and program operational needs and concerns to ensure quality service delivery
  • Provide culturally responsive, strengths-based, and client-centered supports and advocacy.
  • Consistently collaborate with mental health and other social service professionals in various institutional capacities and settings to initiate, refer, and follow-up with services in conjunction with or on behalf of clients.
  • Consistently write case notes and documentation regarding services rendered in a timely manner and in compliance with standards of care of La Familia.
  • Consistently meet productivity and client contact hour’s requirements in compliance with programming and standards of care of Working Wellness.
  • Consistently attend weekly meetings of Working Wellness Staff Meetings/Group Supervision; weekly meetings of individual supervision with the Program Supervisor; monthly ACBHCS meetings; and other meetings or trainings relevant for community or professional stakeholder purposes or professional development, team meetings, trainings, and presentations as needed.
  • Maintain client confidentiality and charts in compliance with Health Insurance Portability and Accountability Act (HIPAA)
  • Comply with all agency’s policies and procedures.
  • Support agency-wide Quality Improvement efforts.
  • Demonstrate a capacity and commitment to provide services to diverse clientele, families, and populations in regards to race; class; gender; sexuality; disability; age; immigration/citizenship status; religion/spirituality; and other social statuses.
  • Participate in peer-to-peer learning by being available for occasional job shadowing.
  • Attend trainings and participate in events relevant to key responsibilities, including IPS trainings and county IPS meetings.
  • Track and regularly report on key metrics for responsible function areas
  • Leads and collaborate on special projects related to agency objectives as appropriate
