SummitRidge Hospital, a place for healing and hope provides quality behavioral health and addiction treatment services, mental health and substance abuse treatment located in Lawrenceville, Georgia, approximately 30 miles northeast of downtown Atlanta. Our inpatient, partial hospitalization and outpatient programs have helped adolescents, adults, older adults and their families. Due to continued growth, we are seeking an experienced Social Services Case Manager with recent behavioral health experience. The Case Manager is a member of the multidisciplinary treatment team. Major functions include conducting psychosocial and assessments, gathering info from family and other providers, identifying discharge needs, and complete discharge planning. Our Case Managers also assist with placement and other discharge planning needs including scheduling appointments for aftercare. Completing documentation per policy and procedure, including treatment plans and treatment plan updates required. Additional job functions include but are not limited to providing clinical social work expertise to hospital wide programs; deliver social services in assigned areas according to patient needs; adhere to standards of patient quality care and work as a member of the interdisciplinary treatment team within guidelines set by the Social Services Department and the individual unit Program Director.
