About The Position

We are seeking a passionate Case Manager to support our DRC Program in Santa Barbara, CA. As a Case Manager at our DRC programs, you’ll play a key role in promoting long-term, self-sustaining skills through vocational training and personal development services. Our Day Reporting Centers in Santa Barbara and Santa Maria, CA provide a community-based alternative to incarceration for clients on parole. CSI uses a curricula-driven, evidence-based model designed to assess the needs of each client to reduce individual risk factors. Participants work closely with case managers and employment specialists to develop individual goals and make positive changes in their lives. At Community Solutions, Inc. Requirements

  • Bachelor’s degree and one year experience in case management or similar field (education may be substituted with experience on a year for year basis).
  • Strong decision-making, organizational, and interpersonal skills.
  • Experience using Microsoft Office.
  • A commitment to working as part of a team.

Nice To Haves

  • Previous assessment and facilitation experience preferred.

Responsibilities

  • Perform client needs assessments within seven (7) days of intake and enter data into CSI data systems on the day the data is obtained.
  • Enter into funder database within five (5) days of client advancement into Services.
  • Actively participate in Client Progress Review (CPR) meetings by presenting new clients, assessments and Individual Service Plan updates on clients as well as discharge planning for clients nearing discharge.
  • Provide and elicit peer support for client issues.
  • Develop and document program plan goals with clients within thirty (30) days of activation and review with client on a bi-weekly basis.
  • Enter goals and relevant client data into Individual service Case Plan and ensure ARMS database reflects case planning efforts and real time progress.
  • Monitor program stipulations, job readiness preparation, and community resource referrals.
  • Facilitate weekly curricula, including entering all new referrals to interventions into the CSI data systems on the day of the referral.
  • Prepare, plan and facilitate individual and group sessions with integrity and fidelity to curriculum/manual.
  • Schedule and conduct make-up sessions as needed.
  • Document group and individual sessions into the agency data systems and funder database on the same day the information is received.
  • Maintain a minimum of 75% completion rate for each intervention facilitated.
  • Maintain a working knowledge of and abide by the Agency’s programs, policies, procedures and code of ethics.
  • Perform other tasks as assigned by supervisor.
