The Young Adult Care and Prevention (YACAP) Case Manager is responsible for intake, assessment, and ongoing support of program participants ages 13 to 29 to identify and address barriers to HIV prevention, HIV testing, and linkage to HIV care when necessary. The Case Manager works under the supervision of the YACAP Program Manager and in collaboration with the medical care team and other staff to ensure the goals and contractual obligations of the YACAP program are met, with a strong focus on community outreach and engagement of young adults and adolescents at increased risk for HIV. The Case Manager is responsible for timely data collection and entry into electronic medical record and reporting systems pursuant to contract needs and program requirements. This person will lead and assist in the implementation of community-based outreach activities, support recruitment and retention efforts, and help develop and implement quality improvement activities while representing the agency in the community. The focus of this role is HIV prevention education, PrEP/PEP navigation, HIV testing and counseling, and linkage to HIV care and treatment services for youth and young adults.
