About The Position

At Dungarvin, we are a national provider of support services for individuals with intellectual and developmental disabilities, including those with mental health, behavioral, and complex medical needs. Our mission, since 1976, is to respect and respond to the choices of people in need of supports, empowering them to live meaningful lives on their own terms. We operate across fifteen states, delivering flexible, high-quality services tailored to individual goals through Medicaid and Community Support Waivers. The Case Manager position assists individuals in identifying and acquiring natural and paid supports to live as independently as possible in the community. This role involves a person-centered approach, supporting individuals in making their own choices and decisions. The case manager acts as an educator, resource, and service coordinator, focusing on coordinating services for vulnerable individuals and helping them navigate the complex service delivery system. This includes ongoing assessment, planning, referral, service coordination, monitoring, consultation, and advocacy. The position requires direct coordination of a caseload and facilitating other services or supports to positively influence individuals' lives, involving contact with various organizations, families, and community members. The case manager is expected to act professionally and represent the interests of the individuals they support.

Requirements

  • Bachelors degree within social work, psychology, sociology or a closely related field or licensed social worker
  • 1 year of experience as a social worker/case manager/care coordinator in a public or private social service agency
  • A valid driver’s license with an acceptable driving record
  • Proof of automobile insurance
  • Knowledge of the local community
  • Good oral and written communication skills
  • Knowledge of the concepts related to self directed services
  • Meet the qualifications of an unlimited driver (requires driving a motor vehicle as a part of her/his Dungarvin work, including vehicles owned or leased by Dungarvin, and/or to drive a vehicle in which a person served is transported)

Responsibilities

  • Assist person served in identifying and acquiring both natural and paid supports which allow them to live as independently as possible in the community
  • Assist the individual in a person centered approach, in a way which supports the person in making his/her own choices and decisions about these supports
  • Function as an educator and a resource as well as service coordinator
  • Focus on coordinating services for vulnerable individuals and assisting them to navigate the complex service delivery system
  • Provide a systematic process of on-going assessment, planning, referral, service coordination, monitoring, consultation and advocacy assistance, through which multiple service needs of individuals are addressed
  • Directly coordinate a caseload
  • Facilitate/coordinate other services or supports, which will positively influence individuals’ lives
  • Have contact with a wide range of organizations, families, and other community members
  • Act in a professional manner at all times, and understand that they represent the interests of the individual they support
