Path of Life Ministries (POLM) is a 501c3 organization serving those experiencing homelessness in the greater Riverside area. Our strategy is to provide a holistic system of care in an integrated, seamless manner for individuals and families, leading them from a place of crisis to a life of stability and self-sufficiency. Position Overview Path of Life Ministries (POLM) Case Managers are responsible for working with clients (shelter guests and/or others enrolled in POLM programs) to assess the client’s assets and needs, and assist the client in developing and monitoring the following: Individual action plans leading toward self-sufficiency. Short- and long-term goals leading toward the successful completion of that action plan, if not exceed expectations. Tools and/or resources and support systems leading toward and exceeding the successful completion of that action plan. In the process of assisting the clients in these areas, the role of a Case Manager is to be an advocate, encourager, and resource in the life of the client. Through their relationship with the client, the Case Manager will also help their client surface, identify, and address less than helpful habits and modes of operation and/or thinking that cause, or may cause, dysfunction in the life of the client, which keeps that client from reaching and maintaining self-sufficiency, and/or places or keeps them and their family in a cycle of poverty, instability, dependency, and/or homelessness.

Requirements

  • Associate or Bachelor’s degree in the Social Sciences, Social Work, Psychology, or related field required.
  • Two years of experience in case management services preferred.
  • Experience working with homeless and mentally ill populations is preferred.
  • A working knowledge and practice of Motivational Interviewing, Trauma Informed Care, and the cognitive approach to providing social services and development services.
  • A working knowledge of the “hidden rules” and motivating factors and dynamics of social class and generational poverty in America.
  • Peer Support and/or WRAP Certified preferred.
  • Computer skills with proficiency in Microsoft Office, Excel and (preferably) Access.
  • Experience working with individuals that are chronically unemployed is preferred.
  • Strong interpersonal skills to engage participants and resource providers.
  • Demonstrated ability to work independently and as an effective team player.
  • Strong written and verbal communication skills.
  • Must maintain and execute confidential information.
  • Highly motivated self-starter and ability to coordinate multiple projects/tasks simultaneously in a high-pressure environment.
  • Ability to work with diverse communities and exercise mature judgment.
  • Problem solving and conflict resolution skills.

Nice To Haves

Responsibilities

  • Maintains Excellent Client Files
  • Tracks Documented Progress and Referrals.
  • Verifies Intake information is complete and accurate and action plan activities run according to program policies and grant guidelines.
  • Performs all client assessments
  • Develop an Individual Action Plan with each client and monitors the Plan weekly.
  • Provides encouragement and assists clients in attaining and/or addressing the following: Employment, Permanent Housing, Medical/Mental Health Concerns, Federal, State and/or Local mainstream benefit assistance, Personal Identification, Counseling/Referrals/Workshops to address Life Skills, Legal Aid, substance abuse issues, and other necessary services that lead the client to self-sufficiency (not an all-inclusive list).
  • Supports Program Directors
  • Provides Manager of Case Management and Shelter Manager with weekly reports of client progress and make recommendations regarding continued client participation and/or exit from program.
  • Attends company case management and operations meetings as well as other outside meetings that relate to job responsibility when requested and/or approved.
