The case manager is responsible for coordinating services for recovery, employment, education, support groups and housing. Essential Duties and Responsibilities Responsible for the case management of assigned individuals. Makes referrals and assists with the coordination of treatment services. Helps individuals identify community agencies, housing opportunities, resources, and supports. Provides assistance to residents with financial linkages and resource management Coordinates educational and/or vocational skill development. Helps each individual develop an informal support system, including natural supports and those requested by the individual. Ensure smooth transitions and minimizes fragmentation of services. Monitors, tracks, and provides support to the individual. Organizes and maintains case records. Assists with intake and admission. Serves on various agency committees. Assists in the implementation of the agency’s policies and procedures. Must abide by the ethical guidelines of the field and any licensing authorities. Facilitates the resident government’s program guidelines. Carries out duties essential to the above stated responsibilities as assigned. Is mindful of and integrates Trauma-Informed Care and its five principles in all duties and interactions with fellow staff, residents, and external partners. Maintains compliance with all required rules and regulations including Federal Confidentiality (42 CFR, Part 2), HIPAA, Medicaid and/or HUD, and any others. Regular attendance at work location.
