About The Position

Well Care Services is a Behavioral Health Clinic looking for Case Managers to join our team. We are hiring a team of case managers that will be working hands on with patients out on the field.

Requirements

• Minimum of two years of experience working with adults with serious mental illness; or an equivalent combination of training and experience.
• Knowledge of serious mental illness, substance abuse, psychotropic drugs, entitlement programs and recovery oriented services required.
• Interviewing, service planning, consultation, public relations, and experience with electronic medical record.
• Computer skills and the ability to type required.
• Good driving
• Must not reflect a total accumulation of six or more negative points within the past three years or a major violation within the past four years.
• Must be able to pass a drug and background check.

Responsibilities

• assessing clients' strengths and needs
• developing individualized service plans
• completing services assessments
• linking consumers with other community resources
• coordinating services including health care and monitoring sufficiency of services and goal attainment
• collaboration with other professionals
• Perform other work as required.
