The Case Manager plays a vital role in supporting individuals with developmental and other disabilities by managing the person-centered planning process and ensuring quality, individualized service delivery. The Case Manager is responsible for coordinating, implementing, and monitoring Individual Support Plans (ISPs), maintaining accurate and timely documentation, and serving as the primary liaison between members, families/guardians, and program staff. This position focuses on service coordination, compliance, and communication—not direct supervision of staff. The Case Manager collaborates closely with Site Managers and Lead Member Support Professionals (Lead MSPs) to ensure member goals are implemented through daily programming and activities.
