About The Position

Reporting to the Director of Mental Health Service, the Case Manager is responsible for coordinating care for students, both self referred and professionally referred from within Mental Health Service. The position works directly with students, families, and providers to assist in arranging and managing a robust set of resources required to meet student health and retention needs. The position will build and maintain collaborative working relationships with various campus resources, resources in the surrounding community and out of state, and pharmaceutical and insurance companies to facilitate access to appropriate services. Further, the Case Manager provides crisis and short-term mental health counseling within MHS as needed. The Case Manager functions as a member of the MHS team consisting of clinical/counseling psychologists, clinical social workers, and licensed professional counselors. The Case Manager should have a deep appreciation for the integrated care model, collaborating with the Medical Clinic, Wellness and Student Support and Lighthouse Program teams. In addition, the Case manager should be committed to advancing the Student Affairs divisional priorities and cultivating a working relationship with Our Lady of the Lake Health.

Requirements

  • Master's in Social Work or mental health counseling from an accredited program.
  • 3 years practice of social work or community counseling in a counseling setting, including some case management experience.
  • Some experience working within a collaborative, multi-disciplinary team.
  • LCSW licensed by the LA Board of Social Work Examiners OR LPC or LMFT by the LA Board of Licensed Professional Counselors
  • Must be able to show proof of license.

Nice To Haves

  • Master's degree with 3 or more years experience.
  • Course work in young adult development and clinical experience working with this population.
  • Experience working in an integrated, collaborative, multi-disciplinary health/mental health facility.
  • Experience working in higher education.

Responsibilities

  • Provides case management services to students, both in-person and through telehealth, within Mental Health Service; assesses individual needs and educates students and families about available resources to access healthcare and support. This position will also assist MHS with short-term counseling and initial assessment services including consultations, triage/walk-in shifts, crisis intervention, and intake for treatment. Case management support may be provided collaboratively to other units within the Student Health Center as needed and appropriate.
  • Manages and regularly updates a comprehensive database of community resources to address a broad range of client needs to include community hospitals, specialized or higher level of care services; medication assistance programs; patient support groups; financial assistance services; disability services assistance; insurance coordination, and government assistance coordination. May participate on campus and community committees and task groups, including the Communicate, Access, Refer, and Education (CARE) Team as needed.
  • Carries out administrative responsibilities including documentation, appointment scheduling, and follow-up in keeping with departmental policies and professional standards. Conducts annual or per-semester utilization reviews for all of MHS, with a focus on increasing efficiencies with termination of care, external referrals, and maximizing in-house group therapy referrals. Produces quantitative and qualitative reports summarizing client care management activities. Participates with MHS team in annual quality improvement studies, chart reviews, and required annual training for the center.
  • Manages client caseload, consulting with fellow providers in Student Health Center as needed for peer review and collaborative care practices. Attends all team, unit and departmental staff meetings, and other meetings as requested. Provides training and supervision to mental health interns through didactic seminars, consultation, and direct supervision as needed.
  • Performs other duties as assigned by Director. Supports broad divisional/institutional efforts including at least 40 hours per year helping with initiatives such as Move-In Day, Welcome Week, Family Weekend, Career Expo, Disability Services Finals Testing, and other special projects.
