Requirements

  • Educational Background: A Bachelor's degree in Social Work, Psychology, Human Services, or a related field.
  • Experience: A minimum of 1 year in mental health or substance abuse treatment settings.
  • Skills: Strong communication, interpersonal, and organizational skills. Proficiency in case management software and electronic health records is essential.
  • Cultural Sensitivity: Ability to work effectively with diverse populations, maintaining respect and understanding of different cultural backgrounds.
  • Completion of a pre-employment drug screening.
  • Completion of CPR/First Aid Training within the first 30 days.
  • Clearance of IDHW Enhanced Background Check.

Responsibilities

  • Initial Client Assessment: Meet with new clients within their first week to evaluate case management needs and set them on the path to success.
  • Customized Treatment Plans: Work closely with clients to develop individualized plans that cater to their unique circumstances and goals.
  • Resource Coordination: Act as a liaison between clients and various service providers, ensuring seamless access to medical, mental health, housing, and employment resources.
  • Ongoing Support: Conduct regular check-ins with clients to monitor progress, address concerns, and adjust plans as needed.
  • Advocacy: Be a strong advocate for your clients, ensuring their needs are met across all service areas.
  • Documentation: Maintain precise and timely records in our electronic health systems, ensuring continuity and quality of care.
