About The Position

Entrust Community Services is seeking a compassionate, organized, and detail‑oriented Case Manager to support individuals and families across multiple programs. This position is based at one of Entrust’s satellite offices, where the Case Manager serves as the on‑site leader, working in partnership with a Program Manager who provides oversight and support. The Case Manager delivers comprehensive case management services—including assessment, planning, coordination, advocacy, and ongoing support—to reduce barriers and promote long‑term stability, independence, and success. This role collaborates closely with Intake and Outreach staff, internal teams, and community partners to ensure services are delivered in accordance with program requirements and Entrust’s mission. The Case Manager builds supportive, professional relationships with individuals and families, assists in developing individualized service plans, monitors progress, and connects participants with community‑based resources. This role requires excellent communication skills, sound judgment, strong problem‑solving abilities, and a commitment to advocacy and ethical service delivery.

Requirements

  • One of the following combinations of education and experience:
  • High School Diploma or GED and at least 3 years of case management experience, or
  • Associate Degree in a related field and 2 years of case management experience, or
  • Bachelor’s Degree in Psychology, Sociology, Special Education, or a related field and 1 year of case management experience.
  • Valid Washington State driver’s license with proof of insurance.
  • Ability to pass a Washington State Criminal Background Check, E‑Verify, and mandatory drug screening.
  • Knowledge of service planning principles, human service systems, and advocacy practices.
  • Ability to develop comprehensive service plans and write clear, accurate technical reports.
  • Strong verbal and written communication skills.
  • Ability to interpret policies, procedures, technical documents, and instructions.
  • Skill in researching community resources and navigating external systems.
  • Strong documentation, organizational, and relationship‑building skills.
  • Ability to maintain professional boundaries while providing compassionate support.
  • Ability to work independently while collaborating effectively as part of a team.
  • Proficiency in Microsoft Office and familiarity with program databases such as Setworks.

Nice To Haves

  • Current First Aid and CPR certification.

Responsibilities

  • Partner with Intake and Outreach staff during intake, eligibility determination, and assessment processes.
  • Develop, implement, and monitor individualized service plans for individuals and families.
  • Conduct periodic reassessments and update service plans based on participant progress and changing needs.
  • Maintain regular, supportive communication with individuals, families, and service providers.
  • Document all case management activities, progress notes, and changes in program databases.
  • Distribute completed service plans, reports, and required documentation to internal staff and external agencies.
  • Establish and maintain collaborative partnerships with Intake, Outreach, and internal and external service providers.
  • Communicate regularly with the Program Manager and team regarding participants awaiting services, transitions, or exits.
  • Coordinate coverage of services during staff absences to ensure continuity of care.
  • Participate in staff meetings, trainings, and collaborative team activities as required.
  • Research, identify, and maintain up‑to‑date knowledge of community resources.
  • Advocate on behalf of participants to promote access to services, supports, and rights.
  • Maintain professional working relationships with community agencies, care providers, and service systems.
  • Exit participants in accordance with program policies, procedures, and documentation requirements.
  • Assist with other program‑related tasks as assigned.
  • Ensure compliance with all programs, licensing, and documentation standards.
