About The Position

BronxWorks helps individuals and families improve their economic and social well-being, providing programs in workforce development, children, youth, families, seniors, and homeless prevention across 60+ locations. The Pyramid Safe Haven, a 125-bed low-barrier shelter operated by BronxWorks, serves chronically street-homeless single adult men in the Melrose section of the Bronx. It follows a Housing First model, offering on-site services such as case management, psychiatric and medical care, substance use counseling, meals, showers, and housing placement support. Referrals are exclusively through NYC’s Street Homeless Outreach Teams. The program aims to build trust and engagement, supporting clients in transitioning into stable, long-term housing.

Requirements

  • Bachelor’s degree in social work or a related field of study.
  • Ability to engage and build rapport with populations with high support needs.
  • Strong oral and written communication skills.
  • Detail-oriented and resourceful with strong time management and organizational skills.
  • Proficiency in Microsoft Office suite and other standard business technology.
  • Excellent map reading skills and ability to navigate public transportation system.
  • Excellent organizational and interpersonal skills.
  • Strong grammar skills.
  • TB Test required with the first 120 days of employment for Shelters and Homeless Outreach programs.

Nice To Haves

  • One-year of case management or related work experience preferred.
  • Bilingual/multilingual preferred but not required.

Responsibilities

  • Independently manage a caseload.
  • Conduct comprehensive intake assessments.
  • Conduct regular field visits to engage and build rapport with chronically homeless individuals.
  • Work with clients to develop goals and service plans.
  • Provide referrals and direct assistance to support clients applying and/or attending appointments For mental health services, medical care, substance treatment, vital documents, benefits, Legal/immigration services, and permanent housing.
  • Document all interactions with or on behalf of clients, monitor and document client progress toward service plan goals.
  • Maintain familiarity with social services and resources available both on-site and in the community.
  • Coordinate client care with other service providers; attend regular case conferences with clients and other service providers, as required.
  • Attend regular supervision sessions with direct supervisor.
  • Drive agency vehicle to and from client locations and appointments.
  • Perform additional duties as assigned.
