About The Position

The Case Manager provides essential case management services to children and families, focusing on facilitating safety, permanency, and well-being. This role involves understanding and supporting the unique needs of families, engaging with various stakeholders, and delivering home and community-based services.

Requirements

  • A bachelor's or master's degree in social work, community counseling, human development, child and family development, applied family and youth services, public health, health sciences, trauma studies, sociology/social services, substance abuse/addictions, early childhood education, or psychology from an accredited college/university.
  • Experience with children and family services preferred.
  • Current, valid Driver's License and Auto Insurance required.
  • Licensure by the Kansas BSRB is required.

Responsibilities

  • Actively seek to understand and support families' lived experience and individual needs.
  • Build and maintain strong engagement with clients, placement providers, DCF, coworkers, and other community partners.
  • Actively share relevant and appropriate information with family and involved professionals.
  • Provide timely responses to all requests.
  • Provide home and community-based services to clients, including observation of caregiver/child interaction and resource coordination.
  • Conduct on-call crisis intervention, safety planning, skill building, and transport clients as needed.
  • Utilize information gathered through assessment processes to advocate for the client's best interest.
  • Provide court testimony, attend school meetings, and coordinate mental health services as part of advocacy efforts.
  • Complete required documentation in a timely and thorough manner, including case logs, comprehensive assessments, court reports, and social histories.
