The Case Manager position at Project Renewal involves providing comprehensive case management services to homeless mentally ill adults. The role focuses on conducting assessments, developing service plans, and coordinating with various service providers to ensure clients receive the necessary support and resources. The Case Manager will also engage in home visits and maintain accurate client records using a digital case management database.

  • Conduct intakes, assessments, and psychosocial evaluations for assigned caseload.
  • Perform a minimum of two home visits per month in scattered apartments throughout the Bronx.
  • Utilize the AWARDS digital case management database to input and update client information in a timely manner.
  • Provide case management services to assist in the creation and adherence to Independent Services Plans.
  • Identify short-term and long-term goals and objectives for clients.
  • Maintain contact with internal and external service providers on behalf of the client.
  • Identify service needs and make referrals for medical, mental health, psychiatric, substance abuse, and other services as needed.
  • Escort clients as needed.
  • Work closely with Occupational Therapist to enhance clients' functional/ADL skills.
  • Participate in clinical team meetings.
  • Perform other duties as assigned by the Program Director.
  • Minimum bachelor's degree in social work, counseling, psychology, human services, or a related field.
  • Direct experience working with homeless mentally ill adults and providing case management/care coordination.
  • Excellent oral, writing, and listening skills.
  • Ability to work well with various deadlines and meet documentation requirements.
  • Ability to interface with clients and all levels of staff.
  • Proficiency in Microsoft Office and familiarity with case management software AWARDS.
