About The Position
The Case Manager at Holcomb Behavioral Health Systems is responsible for providing community-based services to individuals eligible for behavioral health services. This role involves assessing and monitoring resources to help clients live in a stable and safe manner within the community. The Case Manager will carry a caseload of up to 30 clients and will work collaboratively with various stakeholders to ensure effective service delivery and support for clients' recovery.
