About The Position

The Case Manager position at The Sanctuary Foster Care Services in Nassau Bay, TX, is a full-time role focused on providing comprehensive support to children and families involved in the foster care system. The primary responsibility of the Case Manager is to advocate for the physical, emotional, educational, social, spiritual, and cultural needs of children by collaborating with multidisciplinary team members. This role requires the development of individual treatment plans, goals, and outcome measurements within thirty days of a client's admission, followed by regular service plan reviews every 90 days. The Case Manager will maintain accurate and up-to-date case files and conduct monthly visits with each child and family on their assigned caseload to assess needs and provide necessary support. In addition to case management duties, the Case Manager will function as a liaison between children, families, and various community agencies, including the Department of Family and Protective Services (DFPS). This position also involves providing crisis intervention services on a 24-hour basis, alternating weekly coverage with other Case Managers. The Case Manager is expected to represent The Sanctuary Foster Care Services with integrity and professionalism, upholding all confidentiality and abuse reporting requirements as dictated by law. The role requires excellent verbal, written, and interpersonal communication skills, as well as the ability to analyze situations critically and manage time effectively. The Case Manager must be flexible, possess good judgment, and be willing to comply with all state, federal, and local regulations. Participation in in-service workshops, training programs, and agency activities is also required to support professional development and agency growth. This position is not remote and requires the ability to travel between Fort Bend County, Galveston County, and parts of Harris County as needed.
