About The Position
As a Prenatal and Family Medicine Case Manager, you will be responsible for providing comprehensive case management for patients. This includes receiving referral reports from physicians and medical assistants, and responding to patients' needs in person or by telephone. The Prenatal Case Manager performs designated case management within the clinic, assists and carries out the physician's orders, and executes vital clinical tasks in a professional and efficient manner.
