About The Position

The Case Manager position at Hope Network is a vital role that focuses on coordinating services to enhance the recovery and independence of individuals with mental illness. This position requires active engagement in the community, including performing home visits to ensure that clients receive the necessary support in their environments. The Case Manager will work in both residential and clinical settings, providing essential clinical and program consultation to direct care staff. This role is crucial in ensuring compliance with agency standards and the practice model, which is fundamental to the effective delivery of case management services. In this role, the Case Manager will assist consumers in improving their quality of life and maintaining their highest possible level of recovery and functionality within the community. This involves implementing individualized, goal-oriented strategies to obtain necessary services and supports. The Case Manager will also ensure that consumers are actively involved in the Person Centered Planning Process (PCP), facilitating the implementation of their Individual Plan of Service. Comprehensive, ongoing psychosocial assessments will be performed to monitor progress and adapt plans as needed. Additionally, the Case Manager will participate in legal proceedings, including petitioning and testifying in clinical emergencies, and will provide community-based services. Coordination of consumer intakes, discharges, and transfers is a key responsibility, as is communication with all service providers involved in the Treatment Plan. The Case Manager will also facilitate hospitalizations, attend hospital meetings, and coordinate with hospital social workers to ensure seamless transitions for consumers. Monitoring activities such as employment, education, volunteerism, and family caretaking will also be part of the role, along with providing referrals to appropriate community resources.
