About The Position
Under direct supervision, serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. Shifts Available: Full-Time | DAYS | Shifts: 8:30 AM - 5:00 PM | Days: Monday - Friday Telecare's mission is to deliver excellent and effective behavioral health services that engage individuals in recovering their health, hopes, and dreams. Based on the Assertive Community Treatment (ACT) model, STRIDES of Alameda County is an intensive community support service for 120 individuals with severe mental illness (SMI), between the ages of 18-62, who are considered to be at great risk for psychiatric hospitalization.
Requirements
- High School Graduation or G.E.D. equivalent and three (3) years of direct services in mental health or community services; OR Associate’s degree and two (2) years of experience; OR Bachelor’s degree in Social Services
- Must possess at least 1 year of direct service experience providing assistance as a Vocational Specialist
- Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
- Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
