About The Position

Essential Functions Statements: Coordinate with treatment team regarding transition and release process. Participates with DJJ regional transition teams in transition of youth from the program. Directs the transition process in cooperation with case management and transition team members. Coordinate with youth, families, and community resources based on needs identified at the transition staffing. Act as a resource person for all staff dealing with outside systems (i.e. job placement, continued treatment). Works as a contact person for outside job agencies interested in giving second opportunities to troubled youth. Ensure that all guidelines (i.e.: written reports and correspondence) are followed as required by the court and the referring agency. Coordinate with local education / vocational providers for continued education. Conduct post-release follow up to one year. Adheres to the policy and practice concerning issues of creation, editing, transmission, storage and disposition of all Protected Health Information (PHI) of Sequel Youth & Family Youth Services. Requirements

  • Education: Bachelor’s degree in psychology, counseling or related field.
  • Experience: 1 year experience to adjudicated adolescents to include family and group work or equivalent combination of education and experience.
  • Computer Skills: Basic computer skills, including usage of email, Internet Explorer, and print documents.
  • Other Requirements: Fluent in the English language to interact with governing bodies, administration and the community both in person and over the phone.

Nice To Haves

  • Certification is case management preferred.
