About The Position

Case management will have a therapeutic component and evidenced based practices should be utilized. Requirements

BA in social w ork or r e lat e d f i e ld required
A minimum of 3 y e ars r e l e v a nt e xp e r i e nce r e quir e
Exp e ri e nce w orking w i th t e e ns a nd/or y oung adults i nclu d ing tr a n s itional age y out h , spec if ically f o r me r f o s t e r a n d probation y o u t h pr efe rr ed.
Must have a valid driver's license and reliable transportation with a good driving record.
Willing to use own, properly insured, reliable vehicle for the job (mileage reimbursed).
Employment subject to clearances, including background finger printing checks, driving record, tuberculosis testing, and drug testing.

Masters and BBS registered intern preferred.

Responsibilities

Utilize engagement strategies to build rapport, identify strengths and barriers to participant self-sufficiency, and empower youth to achieve goals and permanent independence.

Utilize evidenced based practices to engage youth in participation of services that are client-centered, culturally appropriate, trauma-informed, in order to improve the short/long-term health, safety, and well-being of youth.
Utilize evidence based practices as the modality for crisis management and safety planning, as well as individual and group sessions.
Provide coordination of care with other mental health professionals, primary care providers, and other supportive services.
Coordinate and engage youth in activities in the community to stimulate alternative interests, improve emotional and physical wellbeing, foster leadership development, enhance self-esteem, and support personal growth and satisfaction.

Uphold LifeSpring program compliance and contractual standards through direct monitoring of participants.
