Responsibilities

  • Supervise the work of an assigned team of Case Managers.
  • Complete a Brain Injury Fundamentals Training.
  • Complete training on the Brain Injury Program guidelines and the TBI Act of 1996 and reauthorizations.
  • Review and approve Individual Service Assessments (ISA).
  • Review and approve documentation notes in the individual record.
  • Ensure case managers meet regulatory and company deadlines for the ISAs, notes, required training, etc.
  • Conduct monthly 1:1 supervisory meeting with Case Managers.
  • Monitor and ensure case managers comply with company policies and procedures and all applicable state/federal laws and regulations related to providing Case Management for waiver participants.
  • Implement company progressive discipline as needed.
  • Maintain a small caseload and provide caseload coverage as needed when a Case Manager is on leave.
  • Complete monthly scorecards on each case manager.
  • Provide assistance in keeping individuals Medicaid eligibility current.
  • Other duties as assigned by the State Program Administrator.

Requirements

  • At least two years of Case Management experience provided in Vermont to people with developmental disabilities and/or brain injury.
  • Meet Qualified Developmental Disability Professional (QDDP) qualifications for at least one year.
  • An associate degree in human services or a closely related field and two years of human services related experience.
  • Three years of human services related experience, such as work providing assistance to people and groups with issues.

Nice-to-haves

  • A bachelor's degree in human services, or a closely related field.
  • A bachelor's degree in any field and one year of human services related experience.
  • A registered nurse or a social worker with a graduate degree from a school of social work accredited or approved by the Council on Social Work Education.
