About The Position

The Case Management Supervisor (reporting to the Director of Social Services) has responsibility for the following: ESSENTIAL DUTIES AND RESPONSIBILITIES: Directly supervise the Case Managers Provide orientation and initial training to new Case Managers and Social Work interns Work with Clinical Supervisor, Assistant Director of Social Services and the Case Aide on quality assurance issues including closing of the charts Review daily case notes in CARES and ensure that Case Managers input case notes within 24 hours Monitor incident reports and ensure that follow up case notes are being attached Approve documentation in all charts Identify and coordinate the training and educational needs of the Case Management staff Facilitate groups that will address the needs of the clients Provide support to clients who are struggling to follow up on their ILPs Cover caseloads when necessary Document client engagement in CARES Audit the clients’ charts to ensure compliance Participate in case conferences, team meetings, staff training, community meetings, and staff meetings Perform other duties as assigned.

Requirements

  • Master’s Degree in Social Work (Licensed Master Social Worker), LMHC or equivalent degree.
  • Minimum of 3 years of clinical experience
  • Experience working with the mentally ill and/or homeless population
  • Flexible and comfortable in nontraditional setting
  • Excellent oral and written communication skills
  • Proven supervisory skills (in a supervisory capacity for a minimum of 3 years)
