About The Position

This position is responsible for providing oversight into all case management functions within the SSVF program as well as supervising the Case Managers and Health Care Navigator. This includes performing audits, conducting case conferences, and the development of a case management plan to assist SSVF participants in developing the skills necessary to get stabilized in permanent housing. The position also participates in housing related events for the SSVF program.

Requirements

  • This position requires a bachelor’s degree in social work or a related field or (2) years of experience working with the homeless population.
  • This position requires demonstrated ability in record keeping, good written and verbal communication skills, and knowledge of problems unique to Veterans, and general homeless population.
  • This position requires CPR and First Aid training within ninety (90) days of employment date and recertification as necessary and T.B. testing annually.
  • Requires a valid California driver’s license, a personal vehicle, and ability to meet organization’s insurance carrier guidelines.

Nice To Haves

  • Supervisory experience is preferred.
  • Specific knowledge and experience with military culture, housing first approaches, addiction and recovery, and dysfunctional family dynamics is preferred.

Responsibilities

  • Responsible for performing daily work requirements to achieve established objectives of the department.
  • Develop and maintain positive relationships with landlords, in conjunction with the Case Managers.
  • Provide records and services in compliance with SSVF policies, regulatory and funding source requirements.
  • Plan and assign work for case managers.
  • Lead in annual audit preparation for SSVF grant compliance.
  • Maintain positive relationships with program clients, Volunteers of America staff, the Veterans Administration, and all community service agencies.
  • Maintain NewOrg database and client files to meet grant requirements.
  • Provide crisis management as needed.
  • Complete all discharge paperwork in a timely fashion.
  • Assist with Outreach events and activities.
  • Participate in Volunteers of America and outside training sessions.
  • Perform duties as assigned by the Program Director.
  • Responsible for SSVF case managers’ assistance/administrative duties.
  • Assure a safe, non-judgmental environment for the clients.
  • Maintain positive, professional relationships with clients and staff.
  • Participate in agency and outside training sessions.
  • Regularly update SSVF Policies and Procedures Manual and staff with changes.
  • Voucher/bus pass acquisition and distribution.
  • Work directly with SSVF Case Managers – case concerns, trainings, unity of files.
  • Review SSVF recertification paperwork quarterly or when participant status changes.
  • Review Housing Stabilization Plan that meets the client’s goals and assists in fulfilling the plan.
  • Provide resource information and appropriate case planning.
  • Record keeping and communication.
  • Maintain files on each participant which will include all SSVF eligibility information, short- and long-term plans to maintain and improve self-sufficiency. Document progress and assistance in removing obstacles to set goals. Provide routine documentation of coordination and follow-up of all areas.
  • Provide records and services in compliance with agency policies, regulatory and funding source requirements.
  • Maintain daily, clear, and thorough communication with the Program Director on all areas of responsibility listed in this job description.
  • Responsible for all areas of daily program maintenance in compliance with company policies.
  • Document and report all client concerns and/or infractions that can impact the security of the program.
  • Secure approval for exceptions to standard policy.
  • Notify management of potential workflow problems and resource needs necessary to attain performance standards.
  • Adhere to staffing schedules to provide adequate/safe staffing coverage and to accomplish program objectives.
  • Perform reasonably related duties as assigned by the Program Director.
  • Responsible for training and development of new staff.
  • Supervise Case Managers and Health Care Navigators.
  • Resolve case concerns with and provide training to Case Managers.
  • Conduct ongoing performance review for Case Managers and Housing Specialist.
  • Assist in department orientation and training for new employees as requested to assure quality work outcomes.
  • Identify appropriate ongoing training for both new and existing staff and report needs to management.
  • Serve as part of the program development team by providing information and support for the development of quality operations.
  • Maintain an above average working knowledge of fire, safety, and health standards to assure safe work environment for clients and all personnel.

  • Continually learn and enhance technical and interpersonal skills.
  • Attend staff meetings/assigned training seminars and complete required certifications, i.e. CPR, First Aid, etc.
