The Caseworker / Case Manager provides outreach, assessment, service coordination, and ongoing case management to older adults enrolled in SOEP. The position works in community and home-based settings and uses a person-centered, strengths-based, and trauma-informed approach to engage clients, reduce barriers to care, and connect individuals to appropriate supports and services.
