As a recognized leader in developing and operating supportive housing and community-based programs, The Bridge has 70 years of experience helping New Yorkers with mental illness, including those affected by homelessness, incarceration, and institutionalization. Scope of Position: The Bridge is seeking compassionate and motivated Scattered Site Case Managers to support our mission of transforming lives through comprehensive, evidence-based rehabilitative services. Scattered Site Case Managers provide community-based support to formerly homeless adults with serious mental illness and/or substance use disorders, delivering services primarily in clients’ apartments and throughout the community. Responsibilities include conducting assessments, developing individualized Support Plans, linking clients to medical, mental health, and social services, and helping them build the skills and supports needed to maintain stable housing. Using a person-centered and strengths-based approach, Case Managers also facilitate groups, complete timely documentation, coordinate care with providers, and respond to crises as needed. This role requires empathy, cultural competence, and a commitment to empowering clients toward greater independence and stability.
