Requirements

  • Must possess an Associate or Bachelor degree in Nursing and a valid Registered Nurse license issued by the Kentucky Board of Nursing or a state Nursing Board in the “Compact State Agreement”.

Responsibilities

  • Attends all client related meetings on their assigned floor.
  • Serves as the liaison between the ARNP and outside facilities.
  • Attends medical appointments with clients as needed.
  • Make rounds on assigned units and to hospitals as needed when clients are hospitalized; providing written update to the team.
  • Complete all monthly physical assessments on clients.
  • Completes all nursing assessments (to include but not limited to: Braden, Dehydration, Diabetes, Moses, TD)
  • Attend specialty clinics as needed or requested
  • Ensure immunization records are up-to-date
  • Responsible for writing, monitoring and updating SAMS learning objectives and recording into Key Indicators
  • Write mortality reviews
  • Completes pre-admission assessments for potential clients, all needed paperwork for new admissions as needed.
  • Assist with transition training related to client care
  • Serves on facility committees, attends in-service training programs, participates in drills, and responds to facility emergencies.
