About The Position

Part time opportunity available. This position is a weekend only schedule

Work in collaboration with all healthcare team members to facilitate services to ensure care is appropriate, timely, cost effective and achieves desired results for patients throughout the continuum of care.

Requirements

• Associates degree in Nursing required.
• Registered Nurse required.
• CPR required.
• 3 years of experience in General Medical Surgical/Case Management/Utilization Review/Hospice Care

Nice To Haves

• Certified Case Manager (CCM) preferred.
• Broad based healthcare operational experience.
• Requires strong communication skills, self-directed; dependable: ability to provide education to nursing staff, physicians, patients and families, and ancillary services.
• Ability to merge clinical and financial processes.
• Basic computer knowledge.

Responsibilities

• Coordinates communication among healthcare team members including patient, family, physicians and payers.
• Facilitates interdisciplinary care conferences to provide continuity of care throughout the continuum.
• Identifies appropriate level of care determinations based on admission criteria for setting and reimbursement structures.
• Identification of patient care needs for appropriate screening and referrals.
• Performs patient assessment to include: Clinical status Functional capabilities Recovery level potential Realistic options as alternatives to acute care Patient goals for care
• Develops action plan for discharge planning including specific tasks and timetable.
• Identifications of efficient utilization of clinical resources.
• Tracking, entering and reporting case management variances/avoidable days.
