Scope of Position: The Bridge is seeking dedicated and compassionate Case Managers to join our mission of changing lives by offering help, hope, and opportunity to the most vulnerable members of our community. Case Managers provide essential supportive services to formerly homeless adults diagnosed with serious mental illness and/or substance use disorders, helping them maintain stable housing and achieve greater independence. Responsibilities include connecting clients to benefits, healthcare, and community resources; completing timely documentation; conducting home visits; and providing crisis intervention as needed. This role requires strong organizational, communication, and interpersonal skills, as well as a commitment to delivering culturally sensitive, person-centered care in accordance with agency and regulatory standards.
