About The Position

The Case Manager - Reintegration is responsible for completing initial and ongoing assessments by gathering information from children, their families, and collaterals. This role may involve initial contact with children and subsequent placement, as well as researching and recommending possible kinship placements, giving them priority whenever viable. The Case Manager facilitates and coordinates family meetings, develops and implements case plans and permanency goals with children and families, ensuring that goals are reached within established time frames. They determine and recommend necessary services for the child and family, including mental health, drug and alcohol services, respite care, and supervised or unsupervised visitations/interactions. The Case Manager assists families in establishing a support network and supports kinship and foster families to maintain placements and help children work toward permanency. They prepare families and/or children for moves, reunification, or other appropriate permanency goals, and provide intervention for children and families in crisis situations. The role also involves assessing ongoing case plan goals and recommending reunification, termination of parental rights, or other permanency options. The Case Manager must work with the court system, complete reports for court, participate in court proceedings, and ensure court orders are followed. They maintain written documentation in child/family's case records accurately and within established time frames, and if a child's permanency goal becomes adoption, they complete necessary documents to assist in the adoption process. The Case Manager participates in supervisory and team meetings, sharing pertinent information about children/families' progress, critical trends, barriers, and solutions to achieving outcomes, and works collaboratively with family support workers. They supervise and transport children and families as needed and are part of an on-call rotation to address urgent issues outside of regular working hours.

Requirements

  • Bachelor's degree in social work or human service field from an accredited college or university.
  • License to practice in the State of Kansas preferred.
  • Prefer two years' experience working with children and families.
  • Must be 21 years of age.
  • Must pass a drug screen, MVR, KBI, Child Abuse and Neglect Central Registry Clearance check and an Adult Registry Check.
  • FBI fingerprint check.
  • Lifting Requirements of 50 lbs.
  • Must have a valid driver's license, acceptable driving record and reliable transportation.
  • Mandated reporter.
  • Receives training in trauma informed practice and incorporates knowledge in day-to-day practice.
  • Ability to work flexible schedule including on-call.
  • Understanding of family centered practice.
  • Understanding of the child welfare and state system.
  • Ability to engage families in working relationships.
  • Understanding of child and adolescent development.
  • Demonstrates effective written and verbal communication skills.
  • Ability to travel throughout the State.
  • Consistently exercises discretion and independent judgment in performance of duties.
