Requirements

Active licensure as an RN, Social Worker, Respiratory Therapist, PT, OT, or SLP
2+ years of case management or social work experience in an inpatient setting (rehab experience strongly preferred)
Strong communication, organization, and collaboration skills
Proficiency in Microsoft Office

Nice To Haves

Case Management or Rehab Nursing certification (CCM, ACM, RN‑BC, ARN)

Responsibilities

Lead interdisciplinary care coordination and discharge planning
Identify barriers to progress and help develop solutions with the care team
Document care activities and complete the Individual Plan of Care (IPoC) per CMS guidelines
Facilitate weekly team conferences and family meetings
Communicate patient progress and discharge plans to caregivers
Review Case Mix Groups (CMGs) and collaborate with HIM on comorbidities
Support additional departmental needs as assigned
