About The Position

Heartline Staffing, Strategic Talent Partners with Purple Cow Recruiting, is seeking a Case Manager Registered Nurse (RN) for a permanent opportunity in Tallahassee, Florida. The Case Manager RN facilitates the interdisciplinary plan of care with a focus on medical necessity, level of care, patient throughput, discharge planning, and resource utilization. This role plays a critical part in balancing high-quality patient care with efficient care coordination across the continuum.

Requirements

  • Active, unrestricted Registered Nurse (RN) license for the position being applied for.
  • Florida RN license or valid multistate compact license required (Florida licensure required within 90 days if compact).
  • Associate Degree or Diploma in Nursing required; BSN preferred.
  • Minimum of 3 years of recent (within the last year) Case Manager experience in an acute care setting required.
  • OR
  • Minimum of 3 years of acute care RN experience (Med/Surg, Tele, Neuro, ICU, PCU, or ED) with strong interest in case management.
  • Candidates with home health or insurance case management experience may be considered if they have:
  • – 3 total years of acute care RN experience
  • – At least 1 year of acute care experience within the last 5 years
  • Strong care coordination, communication, and clinical judgment skills.

Nice To Haves

  • Certification in Case Management, Nursing, or Utilization Review preferred.

Responsibilities

  • Perform comprehensive assessments of patients’ psychosocial and medical needs.
  • Develop, implement, and document individualized case management plans of care.
  • Lead and participate in interdisciplinary care coordination and team meetings.
  • Evaluate admissions, observation status, and ongoing medical necessity using approved criteria.
  • Perform utilization management reviews and communicate with third-party payors.
  • Facilitate patient throughput while maintaining quality and efficiency.
  • Identify barriers to care and develop action plans to improve outcomes and processes.
  • Coordinate discharge planning and post-acute services.
  • Serve as a liaison between physicians, patients, families, hospital staff, and external agencies.
  • Advocate for patients with complex needs, including under-resourced populations.
  • Ensure compliance with regulatory, ethical, and quality standards.
