About The Position

The Case Manager is responsible for engaging assessing, planning, and coordinating services for high school students and/or their families, ensuring they receive the necessary support to achieve their academic goals. This position involves working with students from various backgrounds, often those facing complex life challenges, to ensure that their needs are met in a holistic and coordinated manner. The Case Manager will also monitor progress and advocate for clients to achieve the best possible outcomes.

Requirements

  • A bachelor’s degree in human resources, Social Work, Business Administration, or a related field (or equivalent experience).
  • Minimum of 2 years of experience in job development, employment counseling, human resources, or a related field.
  • Strong communication and interpersonal skills, including the ability to build rapport with clients from diverse backgrounds.
  • Excellent organizational and time-management abilities.
  • Problem-solving and critical thinking skills.
  • Ability to navigate complex systems and advocate for clients effectively.
  • Knowledge of community resources and services.
  • Proficiency in Microsoft Office and case management software.

Nice To Haves

  • Experience working with high school-aged youth, such as individuals with disabilities, veterans, or marginalized groups.

Responsibilities

  • Client Assessment: Conduct thorough assessments of students’ needs, including health, social, financial, and emotional factors. Gather information through interviews, surveys, and other methods to develop a complete picture of the student’s situation.
  • Care Plan Development: Create and implement individualized care or service plans based on student assessments. These plans should address immediate needs, set long-term goals, and outline specific steps for meeting those goals.
  • Resource Coordination: Connect students with relevant in-school and community-based resources to support their academic goals. The Case Manager will engage the family as needed Ensure students have access to the necessary services to improve their quality of life.
  • Service Monitoring: Monitor students’ progress toward their goals, adjusting the care plan as necessary. Follow up regularly to ensure that services are being delivered effectively and that the client is receiving the support they need.
  • Advocacy and Support: Advocate on behalf of clients with external agencies, healthcare providers, or other organizations. Help clients navigate complex systems and remove barriers to services or support.
  • Documentation and Reporting: Maintain accurate and up-to-date records of client interactions, assessments, care plans, and outcomes. Prepare regular progress reports for internal purposes and external agencies or funding organizations.
  • Crisis Intervention: Provide immediate assistance in crises, such as medical emergencies, housing instability, or other urgent client needs. Coordinate emergency services and ensure that clients are connected to appropriate resources.
  • Collaboration: Work with a multidisciplinary team, including healthcare providers, social workers, legal professionals, and other case managers, to ensure coordinated care and comprehensive support for clients.
  • Client Education: Educate clients on their rights, available services, and strategies for managing their health and well-being. Guide self-advocacy, navigating social systems, and using available resources.
  • Follow-up and Evaluation: Regularly evaluate the effectiveness of the case plan and make adjustments as needed. Conduct follow-up visits or check-ins to ensure clients continue to receive support and to assess any new needs.
