OVERVIEW The Case Manager for the Preferred Communities (PC) Program will provide support to refugees and immigrants and will work with clients to provide intensive case management. Intensive Case Management entails providing consistent culturally and linguistically appropriate assistance in the domains of basic needs, health, and community as needed for the goals of each client. The goal of the program is to empower clients to become self-sufficient while addressing critical challenges and barriers to their individual success. This position is funded through September 30, 2026. Continuation of employment is contingent on the availability of ongoing funding.

Requirements

  • Great written and oral command of English;
  • Ability to work as a team member and independently, with high level of self-motivation required;
  • Exceptional interpersonal skills, including keen listening ability and relationship building;
  • Proficient in Microsoft Office;
  • Must have a valid driver's license, insurance, clean driving record and personal transportation; and
  • Dedication to the human rights of refugees, immigrants, asylum seekers, and displaced people.
  • Highly adaptable and flexible with the ability to multi-task and prioritize in a demanding work environment;
  • Must pass a criminal background and other screening requirements;
  • Must work in person in the Raleigh office.
  • Ability to travel locally as required.

Nice To Haves

  • Bachelor's degree in social work, counseling, public health, psychology, or a relevant medical field desired;
  • Experience in case management, social work or refugee populations preferred.

Responsibilities

  • Provide culturally and linguistically appropriate intensive case management services, including assessing clients' needs, assisting clients in developing Self-Sufficiency Plans, achieving stability in areas of vulnerability, and developing the ability to live independently of ongoing agency support;
  • Develop resources and build organizational capacity to serve vulnerable clients to create optimal environments for attaining self-sufficiency;
  • Work to connect clients with supportive community networks and support systems to ensure their long-term integration in the community;
  • Ensure that refugees are being served in a linguistically and culturally appropriate manner and provide education and advocacy when necessary;
  • Develop self-sufficiency plans for clients;
  • Referral to agency services to remove barriers to self-sufficiency;
  • Maintain case files;
  • Submit written reports as required;
  • Provide cross-cultural information;
  • Perform other job-related duties as assigned by supervisor.
