Provide case management services in a 19-bed program serving homeless individuals in early recovery. 10 beds are permanent beds targeted for individuals who are chronically homeless and with a diagnosed substance abuse disorder. Provide Needs Assessment and follow up referrals and supportive services to participants in the program. Duties will include intake, case management, daily contact, house meetings, referral, and advocacy. Work to engage client in treatment, refer to programs, and develop/monitor plan to assist in long term recovery. Why Work for SMOC?
